We’ve created a ‘My Life and Legacy’ booklet as our gift to you – a place to record all the things you might need to consider or want others to know about your wishes and arrangements.

This booklet gives you the opportunity to tell your story and make your end-of-life plans so that when the time comes, all the information is in one place. You can either print the booklet and fill it in by hand, or complete and save it digitally, to print at your convenience. The document is yours to keep and update as needed. We don’t retain any of the information you input into the booklet.
